Kevin arrives at the Mill, surprised to find his first patient is a young Muslim woman, Nadia, who has asked for him by name. Kevin is slightly unnerved by her personal questions during his consultation and, not able to diagnose her symptoms, recommends she see Heston for a second opinion. Heston clocks the odd questions Nadia asks about Kevin, but diagnoses stress as the main culprit. At lunch, Kevin notices Nadia waiting outside The Mill, and tries to sneak out. She sees him and forces him to chat to her with a shock revelation: she's his sister. Kevin says he doesn't want anything to do with her - and orders her not to come to The Mill again. Howard returns and is very happy with how Mrs Tembe has looked after the place and painted his office. But when Kevin spills some coffee on the ground, Valerie isn't looking where she's walking and slips on the spill. Slightly bruised and seeking attention, the whole Mill is soon aware of her accident – Valerie thinks she may have broken her knee, shattered her coccyx. Emma thinks it's a mild sprain. Mrs Tembe is most worried about the Health and Safety side of the spill – covering the area with a warning sign, and giving Kevin a dressing down for his disregard. Wanting to ensure this kind of thing doesn't happen again, Mrs Tembe starts placing notes around The Mill – warning of hot water, spill risks and leakages. The staff think this is a bit of an overreaction, but Mrs Tembe will not hear of it, as more posters appear around the surgery. Also, it's time for a new car for the Hollinses - but just how much torque can Karen handle?
